Upcoming Workshops

We will be offering workshops at Northwestern Memorial Hospital on the dates below. Please email liza.rivnay@northwestern.edu if you are interested in joining. All workshops will take place in our downtown Chicago campus and are currently only open to Northwestern Memorial Hospital employees.

Introductory Conversation (SONAR) workshops (1.5 CEUs):
This is a 2 hour workshop with simulated patients to help clinicians respond to cues that a patient may have unmet advance care planning needs and would benefit in further exploring their goals and values.

  • December 16th, 2019 from 2-4pm
  • February 10, 2020 from 2-4pm
  • April 15, 2020 from 8-10am

Champion Conversation workshops (3 CEUs):

This is a 4 hour workshop with simulated patients to help clinicians better communicate with their patients about the patient’s hopes, fears, goals, and values in the face of a serious illness.

  • December 20th, 2019 form 8:30-12:30pm
  • February 5th, 2020 from 8-12pm
  • April 17th, 2020 from from 8-12pm

Publications

The manuscript PREP-CPC: A Multi-Site Intervention for Encouraging Goals of Care Conversations for Hospitalized Patients Facing Serious Illness has been accepted into the American Journal of Hospice and Palliative Medicine in November 2019. PREP-CPC (Preventing Readmissions through Effective Partnerships – Communication and Palliative Care) was phase I of the current PACT project, from 2011-2014.

Expansions and Partnerships

Telligen Partnership
From 2017-2019, PACT has extended its outreach by partnering with Telligen, a Quality Innovation Network-Quality Improvement Organization (QIN-QIO), and a new cohort of hospitals to help patients and their care team better understand what really matters in the face of a serious illness.

Expansion into Outpatient Clinics
Starting in 2017, we have also expanded past the hospital setting into the clinic setting and implemented the PACT program at the Delnor Cancer Center in Geneva, Illinois.

Further expansions within Northwestern
In 2019, we have expanded to the cardiology units of Northwestern Memorial Hospital to better serve the heart failure population.
